  2. What needs to change? For each request, note the page, section, timestamp, or file name.
  3. Why does it need to change? What outcome should the change support?
  4. Is this feedback final and approved by all required stakeholders? [Yes / No]
  5. Anything new that was not included in the original scope? [List separately]

The “why” question is powerful. It stops the process from becoming a list of subjective edits and gives you the reasoning needed to solve the actual problem.

Set the Rules Before You Send the Form

The form works best when the client already knows the revision policy. State the rules in your agreement or project kickoff:

  • number of included revision rounds
  • deadline for submitting consolidated feedback
  • who has authority to approve changes
  • what counts as a revision versus a new deliverable
  • your process for out-of-scope requests

Without those rules, a revision form can collect feedback but cannot protect your time.

Use a calm, practical note:

I have shared the draft for review. To keep this revision round fast and make sure nothing gets missed, please add consolidated feedback to this form by [date]. Once I receive the final notes, I will complete the included revision round and send the updated version.

This positions the form as a client benefit: fewer missed notes and a faster update.

What to Do With New Requests

If a form response includes a new page, feature, audience, deliverable, or direction that was not in the scope, do not bury it in the revision round.

Respond in writing:

That request sits outside the current scope, so I can quote it as an add-on. The included revision round will cover the feedback tied to the agreed deliverables.
